THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Legal Duties
Amtran Medical Transport, Inc. is required by federal law to protect the privacy and security of your protected health information (PHI), provide you with this Notice, follow the terms of this Notice, and notify you if a breach of unsecured PHI occurs.
Uses and Disclosures for Treatment, Payment, and Operations
We may use or share your PHI to provide emergency and non-emergency medical transport services, coordinate care with hospitals and physicians, bill insurance companies, and conduct quality improvement, training, and operational activities.
Other Permitted Uses and Disclosures
We may disclose PHI when required by law, for public health reporting, law enforcement purposes, disaster response, workers’ compensation, or to prevent a serious threat to health or safety.
Substance Use Disorder Records (42 CFR Part 2)
Certain substance use disorder treatment records receive additional federal protection. These records generally cannot be disclosed without your written consent except as permitted by law or court order and may not be used against you in legal proceedings without proper authorization.
Your Rights
You have the right to inspect and copy your records, request corrections, receive an accounting of disclosures, request restrictions, request confidential communications, and revoke authorizations previously given.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Amtran Medical Transport or the U.S. Department of Health and Human Services. You will not face retaliation for filing a complaint.
